INTRODUCTION
    
    The Employees' State Insurance Scheme was introduced in the State of Gujarat in the year 1964. With extension of scheme to various districts, it became necessary to decentralize the work in order to provide better facility to Insured Persons and Employers. Accordingly, on the recommendation of the Regional Board, the ESI Corporation set up Sub Regional Offices at Surat and Baroda in the year 2000.

JURISDICTION

    Sub Regional Office, Surat started functioning from 13.11.2000 in the premises of ESI General Hospital, Surat. There are three districts namely, Surat, Navsari and Valsad under the control of this office, primarily covering the entire South Gujarat. With issuance of Notification for implementation of ESI Scheme, this office has been given the added responsibility of implementation of the scheme to the Union Territory of Daman, Dadra & Nagar Haveli.

    The scheme has been implemented in all the three districts under the jurisdiction of this office with medical service being provided through one ESIS General Hospitals situated at Surat and one ESIC Hospital situated at Vapi and 11 service dispensaries administered by the State Government. In addition there is limited tie up arrangement with Trust Hospitals. The disbursement of cash benefits to the Insured persons are met through four Branch offices.

  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint
